Swansea Learning Technology SupportService Desk

The Service Desk is the function that coordinates all calls in relation to incidents (faults), problems and requests for change. It also manages the database that contains all the assets (IT equipment) that are suppported by SLTS in primary and special schools.

The following forms are available from Swansea Learning Technology Support. When filled in, the data will be sent directly to the Service Desk.

Request for Change To be completed when there is a need to install new software or hardware , when cabling or wiring needs to be installed or moved and for updates to existing software
Incident Report To be completed to inform the Service Desk of a problem concerning hardware, network infrastructure or the internet and e-mail service.
Item(s) Write Off Request To be completed when items in your school need to be written off. This form must be accompanied by a Request for Change (RFC) form.
